Should you need to process a claim please contact one of our Customer Service Representatives at 1-855-867-9915. The CSR will email or fax you a claim form and will be happy to assist you with the process. You can also print or download the claims form. Complete the form along with any supporting documentation related to your claim email it to email@example.com.

We insure against loss of or physical damage to a package contents only. If your item(s) are not in a box the outside material will be considered a shipping container and is not insured.
All claims must be submitted to Ship Sticks within 5 days of scheduled delivery date.
